Phoenix Medical Psychiatric Hospital, a part of NeuroPsychiatric Hospitals, is currently searching for a Masters Level Social Worker/Professional Counselor to join our Social Services team to serve our patients at our psychiatric hospital.

• They will be accountable for overall treatment planning efforts, care conferences with families and providers, family and provider care updates, psychosocial needs and completion.
• Will coordinate and facilitate the development of a discharge plan of care for the patient population which includes accountability for the complex discharge process.
• Will ensure the experience of care for the patient and family is exceptional.

Education: Master's in Social Work (MSW) or Counselor required.

Experience: Two (2) years of social work experience in inpatient behavior health setting or acute psychiatric setting

Licensure: If licensed to practice in the state of Arizona, or currently working towards licensure.
